Evan Perrollaz, Executive Director of Home Care

Evan Perrollaz has almost 30 years of experience in senior living and healthcare services. He possesses extensive expertise in senior living, strategic planning, and business operations.

As the Executive Director of Village at Home, Evan is particularly excited about expanding the district’s services to include home care, addressing a critically underserved need on San Juan Island.

Ingrid Gabriel, Home Care Coordinator

Ingrid Gabriel has resided on San Juan Island for over twenty years. As a dedicated Islander, she committed to serving her community by providing home care services, outreach, and education.

Before submitting and application, ensure that it includes the following information- candidates who submit partial applications will not be considered for a position.

Please Note: San Juan County Public Hospital District No. 1 is not part of San Juan County. We do not adhere to the same schedules, or union agreements.

Community Paramedicine

The Community Paramedicine program bridges the gap between healthcare providers and the essential resources needed for people to stay in their homes. By doing so, it helps avoid unnecessary use of 911 and emergency room (ER) services for preventable health issues, leading to better health outcomes and reducing the call volume for first responders.

Community Paramedicine is designed for patients who would otherwise be unable to receive the necessary support and care at home due to the nature of their care requirements or financial constraints.

The main goals of the community paramedicine program are to enhance the quality of life and health within our community. Our caregivers identify both short-term and long-term interventions to improve patient wellbeing.

We receive patient referrals from Peace Island Medical Center ER (PIMC), 911 first responders, the San Juan County Health Department, Hospice of San Juan, primary care doctors, Compass Health, and family members.

  • Home assessments (e.g. safety)
  • Patient resource need assessment (e.g. food)
  • Chronic disease management (diabetes, CHF)
  • Assisting patients to manage their own healthcare
  • Acute care response to reduce hospitalizations
  • Supportive care for assisted living populations
  • Support for family caregivers
  • Post-discharge follow-up to prevent readmissions
  • Medication reconciliation and compliance
  • Behavioral health follow-up to increase attendance at appointments
  • Assessment with triage and referral
  • Wound management

Requesting Community Paramedicine

Providers: Refer a patient to Community Paramedicine by faxing a referral to 360-378-4489, or calling 360-378-2857.

Patients & Family: Reach out to your primary care provider, or speak with a Community Paramedicine representative about your needs by calling 360-378-2857 ext. 405.

Who Works for Community Paramedicine?

Community Paramedicine providers are certified Emergency Medical Technicians (EMTs) who work directly with Village at Home to deliver essential support services to patients in their homes. Eligibility for Community Paramedicine is determined by patients’ specific circumstances or financial constraints.

While EMTs deliver direct care to Community Paramedicine patients, Village at Home manages appointments, referrals, billing, and other administrative tasks.

This partnership between Community Paramedicine and Village at Home enables the Hospital District to offer comprehensive support services to patients in need.
